How to be a PERFECT student? (Middle School)?

Well I'm an A/B student and sometimes may get a C but not a lot. In math I have a C, Science a B, History a B, Yearbook an A, P.E an A, and English an A. How can I get straight A's? Math is my weakest subject. Can you give me tips on how to be organized and do great?
